Sanitation, Nutrition vs. Vaccines

It is a basic tenet of modern medicine that vaccines get the credit for the elimination of many infectious diseases in the 20th century. However, though these diseases have been nearly eliminated, the case that vaccines should be given the credit is very questionable.

Here is a graph taken from the book Dissolving Illusions and discussed in the article “COVID-19—Lessons from History and The Power of The Terrain.” As you can see, the introduction of vaccines came very late in the process of eliminating these diseases. In some cases the diseases were already practically eliminated when the vaccine was introduced. So, the vaccine gets credit unnecessarily. What really deserves the credit?

According to the article’s author, health experts, epidemiologists and historians attribute the drop in deaths from infectious diseases throughout the 20th century, to:

  • Better sanitation and waste disposal (plumbing, sewer and garbage pick-up)
  • Water treatment
  • Improved education on personal hygiene and public health
  • Improved supply chains (trucking, railroads and interstate commerce) providing improved supplies
  • And quality of food and fortification of food with vitamins and minerals

Two more items probably belong on the list as well:

  • Refrigeration
  • Decreased Poverty
